Do you enjoy helping others get started on their road to recovery?The Terraces of Boise, one of thirteen HumanGood communities honored by U.S. News & World Report as “Best Skilled Nursing Facilities 2024” and “Best Continuing Care Retirement Communities 2024” is hiring an Admissions / Unit Charge RN.** $41 – $46/hr. (DOE) ** Work schedule: Primarily daytime hours with some variability based on AdmissionsA day in the life may include, but not necessarily limited to:

• Working with hospital case managers, the patient, and the patient’ s family to provide placement for skilled nursing services or skilled rehabilitation therapy.
• Communicating with the appropriate interdisciplinary team members (IDT) to ensure a smooth transition from a home or healthcare setting to a HumanGood Skilled Nursing Health Center.
• Monitoring reviews and managing the inquiry process through the Referral Manager System as needed to assist the admissions team.
• Supporting the admissions counselor with inquiries from families, hospitals, assisted living, and other referral sources.
• Maintaining communication and rapport with hospital accounts, accountable care organizations (ACOs), and other referral sources.
• Ensuring utilization of standard admission process via HER system; manages inbound continuity of Care Documents (CCD) via Care quality HER integration.
• Communicating the new admission’s specific needs to IDT, primary care physician, or other providers to ensure Person-Centered quality care is provided and to support discharge goals.
• Attending care conferences, assisting with appointments, and providing other related nursing duties and responsibilities, as needed.

To be successful in this role you would have:

• Current RN license in the state for which applying
• Current CPR certification
• Prior nursing experience in senior care, skilled nursing, admissions, post-acute or sub-acute care is preferred
