Job description: All Metro Health Care, a Modivcare Personal Care Service, is looking for a Per Diem RN Supervisor- Home Care. You will have the opportunity to make a meaningful impact on the lives of our clients and foster the professional growth of our home health aides.

• Pay per Visit

• Sup/Post Hospital: $55


• CD PAP: $40.00


• Reassessment: $95


• Initial Admission: $110


• Visiting Client Territory: Brooklyn borough


• Schedule: Visits planned with the required presence of the patient's aide.

You will...

• Performs on-site supervisory and teaching visits to evaluate caregivers


• Provides clinical orientation to caregivers on assigned home care cases.


• Participates in the training of new nurses and in-services for field staff.


• Performs initial nursing assessments


• Counsels patient/caregiver in meeting nursing and related needs.


• Assigns and coordinates services provided.


• Assists physician in developing a plan of treatment/care and makes necessary revisions based upon patient’s condition/needs.


• Prepares clinical progress notes ensuring any changes in the patient’s condition is reported


• Monitors clinical records for complete and accurate documentation.

• Registered Nurse (RN)


• CPR Certified


• 1 year of Home Care experience required


• 1 year of supervisory or management experience required


• Valid NYS Driver’s License, insurance, registration, and reliable car
